Are you an Occupational Therapist with a passion for delivering outstanding care that brings the 6 Cs (care, compassion, commitment, courage, communication and competence) to life? If so, we want to hear from you.

An opportunity has arisen within our Later Life and Memory Service which provides assessment, diagnosis and treatment for people with suspected Dementia and for people with functional mental health problems that coincides with frailty.

The role would be to complete comprehensive functional assessments with the aim of  supporting people to stay at homestay. Providing links to access support in their local area and education to enable people to live life well. Working in conjunction with local authority teams and  third sector initiatives to enable access to appropriate support. Involvement in post diagnostic groups and Cognitive stimulation therapy.

The post is full time at 37.5  hours per week.

This post is subject to  an enhanced with children’s and adults barred list Disclosure and Barring Service (DBS) Check.  Successful candidates will either have a valid DBS check with the Trust or will be required to pay for the cost of the DBS Disclosure which is £47.50 for an enhanced disclosure.
